Standard Conditions of Probation

The following are standard conditions of Probation that you need to follow until you report to the Probation Office.  You will be instructed on all specific conditions of your personal probation on your appointment date you receive in court.

  1. You will report to your Probation Officer as directed. You will not change your residence or employment or leave the county of your residence without first procuring the consent of your Probation Officer.
  2. Not later than the fifth (5th) day of the month, you will make a full and truthful report to your Probation Officer on the form provided for that purpose, unless otherwise directed by your Probation Officer.
  3. You will neither possess, carry, or own any weapons or firearms without first securing the consent of your Probation Officer.
  4. You will live and remain at liberty without violating any law.  A conviction in a court of law shall not be necessary in order for such a violation to constitute a violation of your probation.
  5. You will not associate with any person engaged in any criminal activity.
  6. You will not use illegal drugs.  You will not use other intoxicants to excess nor will you visit places where intoxicants, drugs, or other dangerous substances are unlawfully sold, dispensed, or used unlawfully. 
  7. You will work faithfully at suitable employment insofar as may be possible.
  8. You will promptly and truthfully answer all inquiries directed to you by the Court or the Probation Officer, and permit your Probation Officer to visit at your home, at your place of employment or elsewhere, and you will comply with all instructions your Probation Officer may give you.
  9. You will submit to random testing as directed by the Probation Officer or the professional staff of the treatment center where he or she is receiving treatment to determine the presence or use of alcohol or controlled substances.

Monthly Report for Probationers

This Monthly Report Form (download below) must be completed each month and must be submitted to your Probation Officer by the 5th day of each month.  You must report the following:

  • Employment information
  • Income
  • Arrest occurring during the report
  • Current address
  • Current telephone number
  • Number of Community Service hours completed
  • Monthly Report
  • Reporte Mensual
The Monthly Report Form may be completed electronically and submitted to the Probation Office. Fill in all required information, sign and submit.

Community Service Information

When sentenced in court for an unlawful offense, probation is often part of the sentence.  In most instances, a probationer will be required to perform a specific number of Community Service work hours.  These hours must be spent at an approved work site.

The form below needs to be taken to the Community Service Work site where you complete your Community Services Hours required by Probation. 

It is the Probationer's responsibility to return the original sheet to the Probation Officer upon completion of the hours in order to receive credit for the hours completed.  Please contact the Probation Office immediately if any problems arise.
